How to Register for Sports at Franklin High School

All potential student-athletes must register on one of the following dates, time, and place:

Fall ’09:
August 12    4-6pm    High School Library
August 13    4-6pm    High School Library
August 14    4-6pm    High School Library

Winter ’09
November 10    4-6pm    High School Library
November 11    4-6pm    High School Library
November 12    4-6pm    High School Library
NOT ON FRIDAY
FIRST PRACTICE DATE November 14 (SATURDAY!)

Spring ’10
February 25    4-6pm    High School Library
February 26    4-6pm    High School Library
February 27    4-6pm    High School Library

All forms must be printed off from the documents section below this article.
Or you can visit the Official School Website and find all the forms under the Athletics link. These forms must be completed prior to registration. Each student must also bring a copy of their report card from their last marking period!

These are the forms necessary for registration:
1.    Physical Form
2.    Athletic Permit
3.    HIPAA Form
4.    Athletic Policy
8.    Emergency Information Form
9.    Copy of Report Card*

*Two failing grades in the previous period results in ineligibility (summer school does NOT count towards eligbility)
Fall Sports – Fourth Marking Period
